Beginning immediately, please complete and submit revised 2026 Agreement versions to be considered for participation in our networks. Earlier Agreement versions that we receive after October 12, 2026 as part of an enrollment request will be withdrawn.

Access Our 2026 Agreement Versions Online
Registered Availity Essentials™ users who can access the Horizon BCBSNJ payer space can follow the steps below to navigate to Agreements for Horizon BCBSNJ and Horizon NJ Health Networks.

1. Log in to Availity Essentials
2. Click on Payer Spaces in the top tool bar and select Horizon BCBSNJ
3. Select Provider Reference Materials under Applications
4. In the new HorizonBlue.com/providers window that opens, mouse over Resources and click Working with Us: Information & Education Resources
5. Click Manuals & User Guides
6. Click Agreements

Delete Prior Agreement Versions
Thank you for deleting/discarding previous Agreement versions you may have downloaded and instead please access the most current Agreement versions accessible online.
