Equinor – Mongstad – follow-up of investigation and order

Objective

The objective of the audit was to verify the implementation of and compliance with the order and non-conformity notices issued in connection with the investigation of a serious incident at Mongstad on 18 January 2023.

Result

Two non-conformities were identified concerning:

• Follow-up of own and contractors' management systems for scaffolding
• Signage and marking in the factory and outdoor area

Furthermore, one improvement point was identified related to emergency response drills and training involving ISO contractors.

Follow-up of order

In the audit, we also followed up on the order issued to Equinor on 5 July 2023. With reference to the order given to BBS on 11 July 2025, we do not consider the order to have been satisfactorily fulfilled in relation to Equinor's responsibility to follow up other participants.

What happens next?

We have asked Equinor to report on how the non-conformities will be addressed, and for their assessment of the improvement point observed. We are also requesting further follow-up of the order issued on 5 July 2023. The reply deadline is set at 30 November 2025.
