Santa Fe CONNECT Case Study Shows $33M in Community Impact

Santa Fe CONNECT is delivering measurable results for the community through coordinated care.

A new case study finds the initiative has generated more than $33 million in combined health care savings and household economic value since 2019. This includes $13.4 million in health care cost savings and $19.7 million in direct value for households.

Launched in partnership between Santa Fe County and the City of Santa Fe, CONNECT has grown from eight programs and 13 navigators into a network of 79 organizations and more than 300 navigators. Together, care is coordinated through a shared, closed-loop referral platform that connects residents to health care and public services.

Key outcomes include:

More than 17,500 referrals supporting over 13,000 residents

Case resolution rates increased from 72 percent to 90 percent

Nearly 3,300 administrative hours saved

The case study highlights how aligning public leadership, community partnerships, and shared technology can improve outcomes while increasing efficiency and accountability.

The findings also point to broader potential. In Santa Fe County, more than 28 percent of residents rely on Medicaid. Addressing health-related, nonmedical needs at scale could generate up to $45.4 million in additional health care savings based on comparable models.

Santa Fe CONNECT demonstrates a scalable approach that other communities can adapt to strengthen care coordination and improve outcomes.
