Alkermes Update to Covered Entities - January 28, 2026

Dear Covered Entity Customer,

As we previously communicated, effective December 1, 2025, Alkermes began using the Truzo platform to facilitate contract pharmacy designations and collection of claims data. Alkermes recently learned that some 340B Covered Entities have experienced challenges accessing the 340B price consistent with our Contract Pharmacy Policy ("Policy") through their respective wholesalers since the transition to the Truzo platform.

We have communicated with Alkermes' wholesalers with access to 340B pricing for Alkermes' products and we believe they have the information necessary to implement the Policy for those 340B Covered Entities who regularly purchase Alkermes' products. We are continuing to work diligently with these same wholesalers to ensure that they possess the necessary information to enable all 340B Covered Entities seeking to purchase Alkermes' products at the 340B Price to do so, consistent with our Policy and the 340B Program, without any challenge.

If you have attempted to purchase an Alkermes product at the 340B price since December 1, 2025 consistent with our Policy and the requirements of the 340B Program and have not been able to do so, please contact us at [email protected] and we will help facilitate the process of a credit and rebill for that purchased product between you and the wholesaler. To support efficient processing of such inquiries, we ask that you provide the following information:

• 340B Covered Entity ID;

• 340B Covered Entity DEA and/or HIN

• NDC of the product;

• quantity purchased;

• original purchase invoice number;

• contract pharmacy name, address, and DEA and/or HIN, if applicable;

• ship-to address;

• invoice date; and

• the name of the wholesaler/distributor from which the product was purchased.
