Premiums are based on your income and the elimination period you choose (90 or 180 days - the number of days that must pass between the first day of disability and the day you begin to accrue your disability benefits). They start at $13 per pay period for those making less than $50,000/year.

90 Day Elimination Period

Below $50,000 - $13/pay period

$50,000 - $64,999 - $27/pay period

$65,000 - $94,999 - $33/pay period

Above $95,000 - $37/pay period

180 Day Elimination Period

Below $50,000 - N/A

$50,000 - $64,999 - $23/pay period

$65,000 - $94,999 - $29/pay period

Above $95,000 - $32/pay period
