OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to [email protected].
Information about health care providers is on the agency website. Click on "Health Care Provider Lookup" under the "Find it Fast" section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
In September 2024 the secretary of health reinstated the chemical dependency professional trainee license of Steven Lee Conner (CO60485119).
In September 2024 the Chiropractic Quality Assurance Commission released Anik J. St. Martin (CH00033965) from the terms and conditions on their chiropractic license.
In September 2024 the secretary of health reinstated the certified nursing assistant license of Gabriel Wyman Vivian (NC60183666).
In September 2024 the Podiatric Medical Board released Brandon D. Nelson (PO60003506) from the terms and conditions on his podiatric physician and surgeon license.
In September 2024 the secretary of health terminated the probation on the emergency medical technician license of Leona Camelle B. Ragil (ES61082916).
In September 2024 the Podiatric Medical Board released Ronald Scott Sorensen (PO00000510) from the terms and conditions on his podiatric physician and surgeon license.
In September 2024 the Board of Denturists released Jorge A. Vizcarra (DN00000259) from the terms and conditions on his denturist license.
In September 2024 the secretary of health charged agency affiliated counselor and licensed social worker James Alan Ghere (CG60956790, LW60921526) with unprofessional conduct. Alleged facts state that in December 2023, Ghere was convicted of possession with the intent to deliver-methamphetamine, a class B felony, and unlawful use of building for drug purposes, a class C felony.
In September 2024 the secretary of health issued Tammy Raye Jacobson a cease-and-desist order after Jacobson was found to have engaged in the practice of veterinary care without a license for approximately 20 years.
In September 2024 the Chiropractic Quality Assurance Commission reinstated the chiropractor license of Bo Robert Mullen (CH60582933).
In August 2024 the Dental Quality Assurance Commission released David G. Myaskovsky (DE00007078) from the terms and conditions on his dentist license.
In August 2024 the secretary of health denied the application of Emily Elizabeth Gewock (CG61444961) for a license to practice as an agency affiliated counselor. In May 2023, Gewock entered a sexual relationship with a former client.
In September 2024 the secretary of health released Douglas Jay Dawson (CP60344743, CO60244012) from the terms and conditions on his substance use disorder professional trainee and chemical dependency professional licenses.
In September 2024 the Board of Nursing released Patricia L. Morrison (RN00130481) from the terms and conditions on her registered nurse license.
In September 2024 the secretary of health terminated the probation on the agency affiliated counselor license of Reggie Renido Parker (CG61278133).
Nevada: In September 2024 the Board of Nursing granted Joshua Michael Hamilton (AP61576290-NP) a license to practice as an advanced registered nurse practitioner with conditions on the license. Hamilton must submit quarterly reports to the Board attesting to his continuing compliance with the Nevada State Board of Pharmacy stipulation and order entered in May 2024 related to his Nevada advanced practice registered nurse license. Hamilton must also attest to his compliance with WAC 246-840-4990 for care provided for Washington patients.
Ohio: In September 2024 the Board of Nursing amended the statement of charges of unprofessional conduct against the registered nurse license and multistate license privilege to practice as a registered nurse in Washington of Andrew Steven Hessler (RN61581779-RN, RN61387555). Alleged facts state in March and December 2023 and in January 2024, Hessler withdrew controlled substances but failed to document administration or waste in patients' records. In March 2024, the Arizona Board of Nursing also summarily suspended Hessler's privilege to practice nursing in Arizona based on findings that in June and July 2023 Hessler withdrew controlled substances but failed to document administration, waste, or return in patients' records.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
