The South Carolina Department of Health and Human Services (SCDHHS) is inviting the public to provide input on the South Carolina Rural Health Transformation Program (RHTP). As part of a statewide rural health assessment, SCDHHS is gathering perspectives from anyone with a connection to rural health in South Carolina, including health care providers, health care professionals and residents.
About the Survey
The survey has two parts:
The survey takes approximately five minutes to complete. All responses are anonymous. No individual responses will be shared or attributed.

The survey closes Oct. 2, 2026.
We encourage all health care providers, health care professionals and community members with a connection to rural health in South Carolina to complete the survey. The information gathered will help inform RHTP planning for years two through five.
