08/21/2026 - RFA-2 submissions announcement - distributed via GovDelivery email

The New York State Workers' Compensation Board (Board) has noticed frequent incorrect submissions of the Request for Further Action by Insurer/Employer (RFA-2 ) eForm.

Please note the request reason "Insurer raising labor market attachment" should only be used to request a Board direction that the injured worker search for work when there is medical evidence supporting a partial disability. This request reason should not be used to suspend or reduce payments directed by the Board pursuant to Workers' Compensation Law §300.23(b).

RFA-2 eForms are also not required on requests that already result in an automated Board action - such as Stipulations and Section 32 waiver agreements which should not be uploaded with an RFA-2 eForm. For issues that do not result in an automated Board action, or if the issue is outside of the RFA-2 eForm, the insurer may submit correspondence. For example, correspondence may be submitted to request cases to travel together.

To request cases be cancelled and combined, an online claims inquiry may be submitted at wcb.ny.gov/contactclaims . The online claims inquiry may also be used to check the status of a previously submitted RFA-2 eForm.
