Project Description:
WW012583 - Permit to operate the Seaview Water Company water supply system, the continued operation of Well No. 2 (S115649), and the installation and operation of one (1) 12" x 8" diameter replacement public supply well (Well No. 3) at the 44 Fairway Avenue Well Field in the village of Ocean Beach, Town of Islip. Well No. 3 will be constructed to a depth of approximately 460 feet below grade (fbg) and equipped with a pumping capacity of 650 gallons per minute (gpm). Existing Well No. 1A (S074807) shall be properly sealed and decommissioned in accordance with NYSDEC Water Well Decommissioning specifications within 365 days of completion of Well No. 3. Notification must be provided to the DEC once the well has been decommissioned. At no time may existing Well No. 1A and Well No. 3 be pumped simultaneously for public water supply purposes. This permit supersedes the former permits for these wells and any subsequent renewals.

State Environmental Quality Review (SEQR) Determination:
Project is an Unlisted Action and will not have a significant impact on the environment. A Negative Declaration is on file. A coordinated review was not performed.
SEQR Lead Agency: None Designated
State Historic Preservation Act (SHPA) Determination:
The proposed activity is not subject to review in accordance with SHPA. The application type is exempt and/or the project involves the continuation of an existing operational activity.
Coastal Management:
This project is not located in a Coastal Management area and is not subject to the Waterfront Revitalization and Coastal Resources Act.
DEC Commissioner Policy 29, Environmental Justice and Permitting (CP-29)
It has been determined that the proposed action is not subject to CP-29.
Opportunity for Public Comment:
