Sentara expands hospital outpatient pharmacy network to Northern Virginia

New Sentara To Home location at Sentara Northern Virginia Medical Center helps patients leave the hospital with medications in hand, recovery underway
Sentara Health has opened its eighth Sentara To Home outpatient pharmacy at Sentara Northern Virginia Medical Center (SNVMC), improving access to medications, enhancing convenience, and supporting better health outcomes for patients in Northern Virginia.

Sentara To Home provides patients with an opportunity to have their prescriptions filled before leaving the hospital, helping reduce delays in treatment and eliminating the need for additional pharmacy stops following discharge.

"The new outpatient pharmacy reflects Sentara's ongoing commitment to healthcare innovation and patient-centered care, offering services that help remove barriers and simplify the healthcare experience," said Jeff Joyner, SNVMC president, SVP Northwestern Market, at the ribbon-cutting on Oct. 14.

The pharmacy is located on the first floor of the medical center across from Market Coffee, and is open Monday through Friday from 9 a.m. to 7 p.m.

Services provided by Sentara To Home

In addition to filling prescriptions, Sentara To Home offers insurance assistance, medication cost evaluations, and support for patients facing financial or other barriers to obtaining prescribed medications. The pharmacy serves hospitalized patients, individuals receiving care through local Sentara Medical Group practices, and Sentara team members.

"By making it easier for patients to access the medications they need before leaving the hospital, we can support safer transitions of care, improve medication adherence, and help patients begin their recovery journey with confidence," said Pharmacy Operations Specialist Ugo Nwachukwu, Pharm.D.

Research has shown that delays in obtaining prescribed medications can negatively affect recovery and lead to avoidable complications. By integrating pharmacy services more closely with hospital care teams, Sentara To Home helps ensure patients receive timely access to the medications needed for treatment and recovery.

The new Sentara To Home outpatient pharmacy also features a secure medication drop box, providing community members with a safe and convenient way to dispose of unused or unwanted medications.

Expansion of pharmacy services across Sentara

The Northern Virginia Sentara To Home location joins outpatient pharmacies at seven Sentara hospitals in Hampton Roads, Harrisonburg, and Charlottesville, with an additional location opening soon in Elizabeth City, North Carolina.

Expanding outpatient pharmacies across the Sentara Health system reflects our commitment to improving access to care and creating a more seamless healthcare experience for our patients.
