New Community of Practice Focused on Evaluating the Impact of CDSME Programs on Social Connection

Commit to Connect (CTC), an ACL initiative, invites applications from aging and disability network organizations for a community of practice (CoP) focused on measuring the social connection impact of chronic disease self-management education (CDSME) programs through pre- and post-test measures. CoP participants will meet five times between March and September 2026. Members will receive no-cost access to collection instruments, technical assistance, and support from CTC staff and a leading researcher, Matthew Lee Smith, PhD.

The CoP is part of CTC's aim to increase the availability of evidence-based programs and services that address social isolation and loneliness.

Interested applicants are encouraged to attend a 30-minute virtual information session on Thursday, February 12, 2026, to learn more and ask questions about Commit to Connect and this opportunity. Register for the information session.
