DOH ISSUES RED PLACARD TO BLAZIN STEAKS-MILILANI SHOPPING CENTER

HONOLULU - The Hawaiʻi Department of Health (DOH) Food Safety Branch (FSB) issued a red placard and immediately shut down Blazin Steaks-Mililani Shopping Center, located at 95-221 Kīpapa Dr., in Mililani, after the operation failed to demonstrate ability to maintain proper temperature control over multiple inspections. The violations include:

• Refrigeration not holding temperature control for safety (TCS) foods at or below 41 F as required; and
• Equipment for cold holding is not sufficient in number and capacity to maintain proper food temperatures.

The food establishment, operated by SZAM INC., received the red placard on Sept. 25, 2026. The establishment must remain closed until all violations listed above are corrected.

DOH is requiring the food establishment to take the following corrective actions before it can reopen:

• Establishment must ensure that all TCS foods are held at 41 F or below.
• Equipment for cold holding is sufficient in number and capacity to maintain proper food temperatures.

Keeping food cold at or below 41 F prevents harmful bacteria from multiplying to levels that may lead to foodborne illness when consumed.

A follow-up inspection has been scheduled for Monday, Sept. 28, 2026.

The DOH FSB protects and promotes the health of Hawai'i residents and visitors through education of food industry workers and regulation of food establishments statewide. The branch conducts routine health inspections of food establishments where food products are prepared, manufactured, distributed, or sold.
