New collaboration brings Bodyport's Cardiac Scale to Sutter Health's Sutter Sync™ program, helping patients monitor fluid status at home while giving care teams timely insights between visits
SAN FRANCISCO and SACRAMENTO, Calif. - Sutter Health and Bodyport Inc. today announced the launch of Sutter Sync™ for patients living with heart failure. Sutter Sync is Sutter's digitally enabled chronic disease management program. The heart failure offering uses the FDA-cleared Bodyport Cardiac Scale to monitor patients' fluid status and support care between visits through Sutter Sync's integrated care model, which combines direct integration into Epic's electronic health record, personalized education and coordinated follow-up support.
Sutter Sync launched in March 2025 to support patients with hypertension, expanded to high cholesterol in October 2025 and began piloting support for diabetes in April 2026. More than 13,000 patients have enrolled, and 90% of participating patients whose blood pressure was previously uncontrolled achieved control within six months. Heart failure marks the program's next expansion.
Heart failure affects an estimated 7.7 million adults in the United States and is a leading cause of hospitalization. Fluid buildup, or congestion, can develop before patients experience symptoms, making timely insight between appointments especially important.
"For patients living with heart failure, important changes can occur between visits. Giving our care teams daily insight into fluid status helps them identify concerning trends, connect with patients sooner and adjust care before a small change becomes a bigger issue," said Dr. Richard Milani, chief clinical innovation officer for Sutter Health.
How it works
Eligible patients are invited to enroll through Sutter's patient portal, My Health Online, based on their risk of hospitalization. Once enrolled, the Cardiac Scale is shipped directly to the patient's home and is designed for easy setup and use. Patients step onto the scale once a day, just as they would a conventional bathroom scale. Each weigh-in captures weight, fluid and other cardiovascular measurements. Bodyport's Congestion Index combines these signals to assess fluid status and flags meaningful changes for the care team to review. The readings flow directly into the patient's electronic health record, where they can be reviewed alongside other clinical information.
Patients can view their weight and fluid measurements on the scale and through Sutter's patient portal app, giving them and their care team a shared view of changes over time. Personalized education helps patients better understand their condition and build healthy habits related to daily routines, diet and medications, empowering them to take a more active role in managing their health.
As with other Sutter Sync programs, Sutter pharmacists monitor trends and coordinate timely medication adjustments between in-person patient appointments. Meanwhile, primary care and specialty physicians can see this progress inside the patient's electronic health record, which they can discuss during the patient's next visit or if the patient reaches out with a question.
"Remote monitoring creates value when it gives care teams timely, actionable information and makes care simpler for patients. Having technology that flags patients who may need prompt attention helps our teams focus first on those at greatest risk. And because the readings flow directly into the electronic health record and are integrated into the Sutter Sync care model, clinicians can track changes, coordinate follow-up support and take action between visits," said John Evans, vice president of digital care for Sutter Health.
"Patients manage heart failure every day, while clinical care has traditionally been informed by periodic snapshots taken during visits. The Bodyport Cardiac Scale turns a simple daily step into objective insight about changes associated with congestion, giving care teams a clearer view between appointments," said Sarah Smith, co-founder and CEO of Bodyport. "Sutter Sync has shown what's possible when connected monitoring is paired with coordinated care and integrated workflows, and we're proud to help extend that model to patients living with heart failure."
