EMS Battalion Chief Jason Hatfield recently completed a six-week Community Paramedicine (CP) training program hosted by the San Francisco Fire Department (SFFD). According to Chief Hatfield, this was an exceptional learning opportunity that provided new perspectives on how we at San Rafael Fire, can expand our approach to patient care.
Community Paramedicine is a discipline that integrates emergency medicine with case work and social work to address the physical, psychological, and social needs of complex patient populations. The core philosophy is to provide the appropriate resources and care for each individual patient-which often means avoiding unnecessary emergency department visits by delivering care at a patient's residence or transporting them to a designated alternate destination that better serves their needs.
The SFFD program is a nationwide leader in CP delivery, and Chief Hatfield felt honored to be one of only three outside department participants selected. The cohort also included paramedics from Chula Vista Fire and UC Davis Fire Department, which provided valuable insights into different regional approaches.
Given the complexity of our patient population in Marin County-particularly among elderly residents and our unhoused community-Chief Hatfield sees significant opportunity to pilot CP initiatives within San Rafael and across the county. He is interested in exploring how we might adapt SFFD's model to address these high-need populations and reduce unnecessary Emergency Department utilization while improving outcomes.
