Washington County Public Health Community Health Survey

Washington County Public Health and their Healthy Community Coalition partners in the county are working together along with other community partners on their 2025 Community Health Improvement Plan (CHIP) to identify and address local health priorities associated with the NYS Prevention Agenda. These local collaborative efforts are crucial for achieving the Prevention Agenda's vision: ensuring that every individual in New York State can attain their highest level of health, regardless of background or circumstances, throughout their lifespan.

This collaborative approach will leverage the efforts and resources of all health organizations in a community toward shared community health goals and improve effectiveness and reduce duplication in the assessment and planning effort. Collaboration between local health departments and hospitals, and alignment between state and local entities, will allow for more effective and efficient health improvement activities.

In order to create this CHIP, Washington County Public Health has surveyed our Stakeholders and collected Health Data from Adirondack Rural Health Network. The next step is to Survey our Community Members to find out what YOU, our Washington County Residents feel is important. It's only a few questions and will only take a few minutes!

The more we know about what our Washington County Resident's needs are, the better we can connect programs to our communities!
