Reminder of Patient-Centered Medical Home (PCMH) Recognition Attestation Deadline

TO: ACHN Participating PCP Groups

RE: Reminder of Patient-Centered Medical Home (PCMH) Recognition Attestation Deadline

The Alabama Medicaid Agency (Medicaid) would like to remind Alabama Coordinated Health Network (ACHN) Primary Care Physician (PCP) groups of the following deadline:

  • PCMH recognition attestation for Fiscal Year (FY) 2027 is due no later than October 1, 2026. Interested PCP groups are recommended to begin their PCMH recognition certification now to avoid missing the October 1st deadline.
  • ALL PCP groups, regardless of the certifying entity from which the PCP group obtained their PCMH recognition, must attest by the specified deadline. Medicaid will notreceive a certified entity listing from any of the PCMH certifying entities to verify a PCP group's certification status.
  • Please Note: Upon receipt of FY 2027 attestation forms and supporting documentation, providers will receive a confirmation email followed by a formal confirmation letter via regular mail.
