Humana Healthy Horizons in Oklahoma Selects Six Nonprofits to Receive Community Impact Awards

OKLAHOMA CITY--(BUSINESS WIRE)-- Humana Healthy Horizons in Oklahoma has awarded six community-based organizations with a Community Impact Award. Totaling $295,500, these awards will be used to address Oklahomans' most urgent health-related social needs, including Food as Medicine programs, access to stable housing and employment opportunities.

These awards are designed in direct alignment with Oklahoma's health priorities and the needs of Humana's Medicaid population in the state. Recent Health Risk Assessment data reveal that 60% of Humana Healthy Horizons' participating members reported experiencing financial strain, and 40% identified as experiencing food insecurity.

The six organizations selected for this award cycle are:

• BeHeard: Operating in Northeast Tulsa, BeHeard is Oklahoma's only mobile drop-in center dedicated to increasing access to basic needs and supportive services for people experiencing homelessness. Through a fleet of specialized trailers and buses, BeHeard delivers essential services including showers, laundry, clothing, haircuts, case management, job program support, and hygiene stations directly to those who cannot access traditional shelter facilities. BeHeard's award will be used to expand services in Tulsa and Oklahoma City.
• FreshRx: FreshRx is a pioneering initiative addressing the critical public health challenges of chronic disease, poor nutrition, and limited access to fresh food in Tulsa Oklahoma. FreshRx prescribes fresh, locally sourced, nutrient-dense foods as a clinical intervention for individuals with Type 2 diabetes and prediabetes.
• OKC Food Hub: OKC Food Hub is committed to improving health outcomes and strengthening local economies by expanding access to fresh, locally sourced produce-especially in low-income, low-access (LILA) communities. Their Farm to Food Assistance program partners with fellow nonprofit, religious and tribal organizations to facilitate direct purchases of fruits and vegetables from Oklahoma farmers.
• RestoreOKC: RestoreOKC addresses persistent health disparities in Northeast Oklahoma City through its integrated, place-based strategy "HERE" (Health, Education, Residential housing, Employment). Their award will sustain and expand community health initiatives such as the Food Loyalty Program and collaborative health events with fellow local partners.
• A Table in the Wilderness: A Table in the Wilderness is dedicated to addressing the urgent public health crisis of childhood obesity and diabetes in rural Oklahoma, where rates of these conditions are among the highest in the nation. The At the Kids Table program leverages S.T.E.A.M. (Science, Technology, Engineering, Art, and Mathematics) concepts to engage children and families in hands-on cooking classes. These classes promote nutrition literacy, foster healthy eating habits, and strengthen family bonds, aiming to prevent obesity and diabetes from childhood onward.
• Youth Medical Mentorship: YMM is dedicated to advancing workforce development, healthcare access, health literacy, and improved health outcomes throughout Oklahoma. YMM focuses on increasing representation and retention of healthcare professionals, particularly among historically marginalized and underrepresented groups.

Community-based organizations (CBOs) have direct knowledge of the immediate health-related issues their communities face and are trusted resources that can help surmount barriers to care.
