Medicare Telehealth Flexibilities Extended Through 2027

Congress has extended Medicare telehealth flexibilities through December 31, 2027. This is a Medicare fee-for-service update only. There are no changes to Medicaid policy.

Telehealth remains allowed for FQHCs through December 31, 2027.

Medical Telehealth
FQHCs should continue billing non-behavioral health telehealth services under HCPCS G2025. These services are paid at the FQHC Medicare telehealth rate, which is lower than the full FQHC PPS rate and remains approximately $98 per visit under current policy.

Audio-Only Telehealth
Audio-only telehealth services remain permitted through December 31, 2027. There is no near-term change to current workflows.

Behavioral Health Telehealth
Behavioral health services furnished via telehealth continue to be paid under the FQHC Prospective Payment System all-inclusive rate. The in-person visit requirement for mental health telehealth services is delayed until January 1, 2028. Providers do not need to meet the in-person requirement before 2028, and existing telehealth workflows may continue through the end of 2027.
