Judge Herren has adopted revised forms to be used for a Final Pretrial Order, Hearing Scheduling Order, and Report of Parties' Planning Conference. For proceedings with Judge Herren, please use his current forms linked under "Judge-specific Guidelines and Forms" on the Court's website.
