Effective September 3, 2025, Official Form B121 - Statement of Social Security Number (aka Statement of SSN) must be filed at the time of filing the voluntary petition (see F.R.B.P. 1007(f)), and as a separate document (e.g. not included within the petition PDF). In CM/ECF, the event will be located under Bankruptcy > Other > Form 121 - Statement of SSN.
Beginning October 1, 2025, an order will be issued if the Statement of SSN is not timely filed. The order will state that the bankruptcy case will be dismissed without further notice or hearing if the debtor does not file the Statement of SSN within 7 days of the date of the order.
