Many older adults and people with disabilities living in subsidized housing have unmet needs for supportive services that most property owners or housing managers cannot identify or effectively coordinate. Service coordination is one strategy that housing providers can pursue, so individuals have professional assistance with accessing supportive services that promote health, wellness, and housing stability. Pursuing service coordination also develops local partnerships to bring community programs and supports on site.
Join this Housing and Services Resource Center webinar to learn how the National Church Residences partners with disability, aging, and health organizations to coordinate services, improve enrollment in programs, and increase available community supports. Presenters from the National Church Residences will discuss how they scaled service coordination across subsidized housing properties and leveraged community partners to deliver on-site programs.

Launched in partnership between the U.S. departments of Health and Human Services (HHS) and Housing and Urban Development, the Housing and Services Resource Center (HSRC) fosters cross-sector partnerships between the organizations and systems that provide housing resources and homelessness services, health care and behavioral health services, independent living services, and other supportive services. The HSRC is part of an interagency initiative to streamline and expand access to affordable, accessible housing and the critical services that make community living possible. Within HHS, this partnership is led by ACL and includes the Centers for Medicare & Medicaid Services, the Substance Abuse and Mental Health Services Administration, and the Office of the Assistant Secretary for Planning and Evaluation.
