Close to Home: Zhanna Panchuk Making a Difference One Home at a Time

During an IHE, clinicians identify chronic conditions, close gaps in care, and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs.

For Signify Health nurse practitioner Zhanna Panchuk, becoming a clinician has always been about making a difference in people's lives. After more than a decade in home health, she continues to find purpose in supporting health plan members where they feel most comfortable - in their own homes.

Zhanna's journey began shortly after she immigrated to the United States from Ukraine in 2008. "I wanted to be in a profession where I could truly help others," she says. "Nursing gave me that opportunity. I could be there for people during difficult times and know that what I was doing mattered." She started as a home health nurse while completing her degrees, eventually earning her master's and nurse practitionerlicense.

When she learned about Signify Health, the role felt like a natural fit. "I already knew how rewarding it is to support people and provide them help in their homes," she says. "I liked the idea of doing that while also focusing on assessments and screenings, like the In-Home Health Evaluation (IHE), that can have such a meaningful impact on someone's health."

One of the things Zhanna values most about her role is the time she gets to spend with each member. Visits often last 45 minutes to an hour, much longer than many primary care appointments. "That extra time lets me really listen to members, hear their stories and understand what they're going through," she says. "Sometimes they share challenges like anxiety, depression or the loss of a loved one. Other times, it's about safety in the home or making sure medications are managed correctly. Every visit is a chance to support someone in a meaningful way."

Zhanna also performs important screenings during her visits, including diabetic eye exams. Using a special camera, she takes images of the retina that are later reviewed by eye specialists. "This screening can detect diabetic retinopathy, which is a serious condition that can lead to blindness if not caught early," she says. "For members who may not be able to see an eye doctor regularly, it's a way to close that gap. We can identify risks sooner and help them get the support they need."

In addition to screenings, Zhanna often helps identify resources that improve members' daily lives. From medication checks to referrals for home safety or pest control, she looks at the full picture. "It's about more than just numbers on a chart," she says. "It's about understanding the environment someone lives in and making sure they have the right support system in place."

Some moments stand out as especially meaningful. Zhanna recalls visiting a member who was managing a health concern and already had a plan in place with his cardiology care team. Seeing how well-supported he was reassured her that he was on the right track. "It was encouraging to know that the monitoring he had in place was working for him," she says

Looking back on her journey, Zhanna is grateful for the opportunity to be part of Signify Health and to bring meaningful support directly to members' homes. "This work reminds me every day why I became a nurse," she says. "It's about listening, building trust and helping people live healthier lives.
