Falls Community Health Highlights Affordable Care Options

Falls Community Health is joining community health centers nationwide in recognizing National Health Center Week, Aug. 2-8, by highlighting the role it plays in providing affordable, comprehensive health care for individuals and families in Sioux Falls.

Falls Community Health offers primary medical care, dental care, behavioral health services and care coordination for patients of all ages. The clinic serves insured, uninsured and underinsured patients and accepts most private insurance plans, Medicare and Medicaid. Patients who qualify may receive care through a sliding-fee discount program based on household income.

"National Health Center Week provides an opportunity to raise awareness about the services available through Falls Community Health and the importance of accessible health care in our community," said Joe Kippley, public health director for the City of Sioux Falls. "Our team is committed to providing quality, comprehensive care while helping patients navigate their health care needs."

Falls Community Health, located at 521 N. Main Ave. in Sioux Falls, offers the following services:

• Team-based primary care for acute and chronic illnesses
• Preventive care, physical exams and health screenings
• Well-child visits and immunizations
• Behavioral health counseling
• Dental care for children and adults
• Laboratory and X-ray services
• HIV/AIDS early intervention services and case management
• Family planning services
• Case management, care coordination and patient navigation
• Transportation assistance and connections to community resources
• Affordable medication programs

In addition to its downtown Sioux Falls clinic, Falls Community Health operates school-based health clinics and a mobile dental bus. Each clinic is staffed by an advanced practice provider, a nurse and dental staff members and provides care to students with parent or guardian consent.
