Horizon Specialty Care (HMO C-SNP) Form Now Available

We previously announced that beginning January 1, 2027, Horizon Healthcare of New Jersey, Inc. (Horizon) will offer Horizon Specialty Care (HMO C-SNP), a Medicare Advantage chronic condition special needs plan designed to support members with certain chronic conditions in select NJ counties.

You will need to complete the Horizon Specialty Care (HMO C-SNP) Verification of Chronic Condition (VCC) form for patients who join this plan. You must submit this form to Horizon within 60 days of a member's enrollment in Horizon Specialty Care (HMO C-SNP) to verify that beneficiary's diagnosis of having one or more qualifying conditions. If we do not receive a completed VCC form, the member will be disenrolled.
