Jefferson and Brown's Shoprite Supermarkets Partner to Break Down Barriers to Healthcare in Vulnerable Communities

In-Store "Healthy Together Hubs" to Address Urgent and Preventive Care Needs in Lower Northeast and North Philadelphia

Jefferson has partnered with Brown's Super Stores, the United Way of Greater Philadelphia and Southern New Jersey and a coalition of local leaders to make healthcare more accessible to the lower Northeast and North Philadelphia communities. The initiative creates "Healthy Together Hubs" that combine Jefferson's community outreach, engagement, screening and health education within two busy grocery store settings to address both urgent health-andwellness-related needs and preventive care.

A Jefferson community-based care team, comprised of a clinician, health educator and community health worker, will be stationed at "Healthy Together Hubs" twice a week inside both the ShopRite of Aramingo and Cheltenham stores operated by the Brown family to perform free health screenings and provide connections to local social services. Health screenings will examine blood pressure, A1C and cholesterol for signs of hypertension, diabetes or other serious medical conditions. Community members are offered the opportunity to make in-person appointments with Jefferson providers. The ShopRites of Aramingo and Cheltenham are in the vicinity of five Jefferson-affiliated in-person urgent care/outpatient care offices and Jefferson Frankford Hospital and Jefferson Einstein Philadelphia Hospital. The on-site care team members are skilled at navigating virtual primary care and home-based primary care options to further enhance access to preventive healthcare. Jefferson community health workers and health educators will provide accessible, easy-to-understand education on nutrition and exercise and identify resources to assist with nonmedical needs like access to transportation that are shown to improve health outcomes.

The "Healthy Together Hub" initiative is coordinated by the Philadelphia Partnership for Nutrition and Health (PPNH), a coalition of more than 20 leaders from business, healthcare and the nonprofit sector focused on advancing equitable access to nutrition and preventive care. PPNH serves as a convener - bringing the right partners together, aligning incentives and helping design and scale initiatives that are both community driven and economically sustainable. PPNH was founded by Ken Frazier, former Chairman and CEO of Merck, and is a strategic initiative of the United Way of Greater Philadelphia and Southern New Jersey (UWGPSNJ).

The Jefferson "Healthy Together Hubs" launched on Tuesday, June 9, starting with the ShopRite of Aramingo location. They will operate 11 a.m. to 3 p.m. on Tuesdays and Fridays weekly at the Aramingo store and 11 a.m. to 3 p.m. on Wednesdays and Fridays at the ShopRite of Cheltenham.
