After it self-disclosed conduct to OIG, SIXRSIG d/b/a Hospice of San Antonio (HSA), San Antonio, Texas, agreed to pay $256,138.68 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that HSA submitted claims for services provided by an unlicensed registered nurse.
