Outside activities approval required per BOR policy

As the fiscal year begins, The Office of Human Resources would like to remind employees of the Outside Activity Approval Request Form available via my.gcsu.edu. The purpose of this form is for employees to disclose any potential, actual or apparent conflicts of interest before engaging in compensated outside activities that relate to their expertise or responsibilities as a USG employee. Such activities may include, but are not limited to, consulting, teaching, speaking, offering services, etc. The form has been recently revised only to update the link to the applicable BOR policy following a website platform migration completed by the USG. Please refer to corresponding USG policy, BOR 08.02.18.02.03, for additional information.

After logging into my.gcsu.edu, find Forms from the Service Apps menu, then scroll down and locate University Required Forms.
