When a woman relocated from California to Nassau County, Florida, to be closer to family, she arrived without insurance, seven months short of Medicare eligibility and managing multiple chronic conditions with no clear path to care.
A Medical Mission at Home event changed her trajectory. In a single day, she received an EKG, pulmonary and neurology consultations, physical therapy, and a referral pathway connecting her to Ascension St. Vincent's Mobile Health Outreach Ministry for ongoing primary care. "This is the best thing I've ever done in my life," she said. "This care is unlike anything I've ever experienced before."
Hers is one of 376 stories like it. This spring, Ascension Florida hosted Medical Mission at Home events in Jacksonville and Pensacola, reaching 376 patients through 707 volunteers and 42 community partners. Across both events:
