Spearheaded by NashvilleHealth, the Heart of Nashville project integrates clinical care with essential wraparound services - including access to healthy food, transportation, and fitness opportunities - to improve health outcomes for residents with hypertension. The initiative focuses on North Nashville, which Metro Nashville designated as the city's first Wellness Opportunity Zone in 2023 to address long-standing health disparities. Currently, 245 patients with uncontrolled hypertension from Matthew Walker Comprehensive Health Center are enrolled, receiving coordinated clinical care and social determinant of health (SDOH) resources. Ongoing project evaluation will measure the program's clinical effectiveness and serve as a framework for future cross-sector collaborations between clinical, municipal, and community organizations.
