Jury Service Questionnaires Are Being Mailed Soon - Here is What You Need to Know

Jury Service questionnaires are being mailed soon to Pinal County residents. If you are selected as a member of the jury pool, you will receive a jury questionnaire.

This questionnaire is not a jury summons. Juror names are drawn randomly twice a year from lists received from the Arizona Department of Transportation Motor Vehicle Division and Pinal County Voter Registration. Jury Questionnaires are then mailed to those individuals whose names are randomly drawn. All questions on the questionnaire need to be completed and then returned either online (preferred) or by mail.

To be excused from jury duty due to health issues, here is the Medical Form required to be excused from jury duty. To be considered for any excusal from jury service, mail in the required documentation with your questionnaire, or you may upload documents online. Do not send the paper questionnaire if you complete the survey online. All prospective jurors are encouraged to fill out the questionnaire online as it:

  • What is this? A--You are being asked to answer questions to determine if you meet the qualifications to be a prospective juror.
  • How can I get out of jury service? A--Answer the questionnaire honestly and return any documentation requested with the questionnaire. Once the proper documentation is provided, you may be excused.
  • Why did I get this questionnaire? A--We pull prospective jurors from DMV records and Voter Registration records. If you have an AZ Driver's License or ID or if you are registered to vote you can receive a questionnaire.
  • When will I need to report for jury service? A--If you are qualified to be a prospective juror, you will receive a Summons letter with a date, time, and location for reporting. Summonses are typically mailed out about a month in advance.
