New Modifier 25 Documentation Resource

APMA added a new educational resource to its -25 Modifier Resources page to help members better understand documentation expectations when using a -25 modifier to report an evaluation and management (E/M) service performed on the same day as a procedure. Using a common plantar fasciitis evaluation and injection scenario, the example shows how documentation can support a significant, separately identifiable E/M service where separate payment may be appropriate. This resource is intended to help members strengthen their documentation and support accurate coding, thereby ensuring fair and sustainable payment.
