This year brought exciting momentum across our network. CHCANYS hosted dynamic learning sessions that sparked new ideas, shared practical resources to strengthen day-to-day practice, implemented hypertension control improvement projects with small cohorts of health centers across the state, hosted a dynamic and well-attended maternal health symposium and provided foundational training to help health centers build a response to intimate partner violence (IPV). These initiatives equipped teams with actionable tools and fresh insights to advance hypertension care and other priority focus areas to close gaps and improve population health outcomes. These 2025 highlights celebrate CHCANYS' commitment to partnering with community health centers to advance high-quality care and create lasting, positive impact in the communities they serve.
CHCANYS Blood Pressure Control Initiatives
CHCANYS continued work on two collaborative CDC/DOH grants to address hypertension control in communities across both East and Central Harlem in New York City, and the East Side of Buffalo. These projects are built around both intense one-on-one technical assistance to health centers in the designated communities as well as learning collaboratives and ongoing partnerships between the participating health centers and community partners who address patient's non-clinical needs.
CHCANYS also launched the Achieving 100% Blood Pressure Control Initiative with a small cohort of health centers from across the network. This multi-year project aims to equip health centers with tools and strategies they can leverage to improve blood pressure control through individualized coaching and collaborative learning models. As part of this work, CHCANYS has developed a data-driven tool that enables health centers to identify others with similar clinical characteristics such as region, site capacity to explore how exogenous factors inform performance on BP control, and to promote opportunities for cross-collaboration and learning. Future enhancements to the tool will enable health centers to benchmark themselves against each other with greater precision to promote evidence-based strategies focused on elevating delivery of hypertension treatment and care across the state.
Population Health Resources
CHCANYS' commitment to supporting health centers as they enhance and transform the way care is delivered was exemplified through our dissemination of curated resources, practical tools, and evidence-based strategies focused on improving quality, access, and outcomes. The resources shared in 2025 are designed to help health centers lead with confidence and positively impact patient health outcomes by implementing new care models, optimizing workflows, and strengthening care delivery.
Maternal Health Symposium
CHCANYS hosted a dynamic virtual Maternal Health Symposium on cross-sector collaboration to improve access and outcomes. This event focused on equipping health center staff at all levels with the knowledge, skills and strategies needed to enhance maternal health care delivery, strengthen data collection efforts and supporting implementation of sustainable, evidence-based models of care.
The symposium created an interactive space where health center professionals, state and local health officials, community organizations, and individuals with lived experience could collaborate to advance knowledge on maternal health treatment and services by identifying and addressing the systemic gaps that hinder access to timely care.

Building a Response to Intimate Partner Violence
In 2025, CHCANYS partnered with Azara Healthcare and other primary care associations to build new CPCI data-warehouse tools to help health centers develop a robust response to Intimate Partner Violence (IPV). This work produced a DRVS safety-intervention measure and a dedicated IPV dashboard supporting IPV planning and targeted action based on patient demographics and risk factors. The tools are designed for seamless integration into team-based workflows, strengthening risk screening, and clinical quality improvement. In addition, CHCANYS delivered several state-wide webinars providing health centers with practical guidance on developing a response to patients exposed to IPV. These materials are available on the CHCANYS website in a dedicated resource hub. Building on this foundation, CHCANYS is developing additional learning sessions to be offered in 2026 to help health centers deepen and accelerate their IPV response.
Webinars/Learning Sessions
CHCANYS has presented several sessions in 2025 in support of remote physiological monitoring (RPM). Presented in collaboration with Ingenium Digital Health Advisors, these sessions have provided a framework for planning, implementing, optimizing, and sustaining an RPM program. These live webinars were also accompanied by the "Making RPM Workbook", an interactive guidebook designed to support relevant health center team members from planning to execution of RPM programs. Through these resources, community health centers have become better positioned to initiate, sustain and spread RPM programs for the benefit of a multitude of patient populations that present with, and aim to more optimally manage, chronic conditions and co-morbidities.
CHCANYS' Medical Assistant Apprenticeship Program (MAAP)
In 2025, five apprentices successfully completed CHCANYS' Medical Assistant Apprenticeship Program (MAAP), each fulfilling 2,000 hours of on-the-job training and 144 hours of online coursework. Through this comprehensive training model, apprentices developed core clinical and administrative competencies, strengthened professionalism and patient-centered skills, and demonstrated their ability to meet the demands of the medical assistant role within Community Health Centers. This achievement reflects the dedication of MAAP coaches (experienced MAs who provided hands-on coaching, mentorship, and professional guidance throughout the apprenticeship) as well as the essential contributions of Community Health Center staff whose administrative and operational support made the successful implementation of MAAP possible. Together, these collective efforts highlight the impact of apprenticeship as a sustainable workforce development strategy and a continued investment in the future of Community Health Centers.
Data Governance Initiative with the Data Stewardship Work Group
The NYS-HCCN completed a multi-part webinar series on data governance and quality improvement for health centers. As technology becomes central to care delivery, strong data systems and good governance are critical for transparency, trust, and better decision-making. This series provided practical insights on governance structures, improving data quality, and leveraging analytics and visualization tools to enhance patient outcomes and operational efficiency. To further support efforts to strengthen data-informed cultures within our membership, CHCANYS worked with John Snow, Inc. and the Data Stewardship Work Group to develop a Data Governance Policy and Procedure Template.
Transitions of Care Case Study
The NYS-HCCN developed a case study highlighting how two health centers optimized transitions of care activities between emergency or inpatient discharge and primary care. Through the establishment of dedicated teams, formalized workflows, and developing strong partnerships with hospitals, health homes, and vendors, these health centers leveraged technology and the power of collaboration to transform patient outcomes. Despite differences in size and service areas, both health centers demonstrated that structured processes and strategic partnerships are key to successful transitional care management.
