In-home primary care provider HarmonyCares recently announced that it had been named as one of the Medicare Top ACO REACH performers for 2024, as reported in data published by the Centers for Medicare & Medicaid Services. HarmonyCares successfully maintained its position as the second-best performing participant in the ACO REACH program - achieving a 25.8% net savings rate and demonstrating itself as one of the highest performers in the High Needs Track from among the 115 accountable care organizations currently enrolled in the program.
HarmonyCares Chief Executive Officer Matt Chance gave additional context on the importance of this announcement in the company's press release, stating "These results validate our approach to patient care.
