Should You Buy Baxter Stock For Its Cash

Baxter International (BAX) stock offers a free cash flow yield of 9.1%, compared with 4.2% for the median company in the S&P 500. A yield that high means one of two things: a business on sale, or a company investors expect to get smaller. During its October 29, 2026 call, management raised its sales outlook for the year but also named headwinds for 2027. So how much of that cash can a shareholder count on?

How Much Cash Does Baxter Make For Shareholders?

Baxter produced $1.13 billion of free cash flow in the twelve months to September 30, 2026, up from $0.23 billion a year earlier. That earlier period includes four months before Baxter sold its Kidney Care business on January 31, 2025. Like profit, that cash belongs to shareholders whether or not it is paid out. Over time, a share price tends to catch up with the cash a company keeps, especially when that cash is growing.

Baxter sells medical products to hospitals, ranging from Infusion Systems and injectables to Advanced Surgery and Drug Compounding. Capital spending took 30.9% of the cash its operations brought in, leaving the rest as free cash flow.

The company also carries heavy debt, with net debt equaling 54.6% of its market value. Counting that debt as part of what a buyer pays puts the yield on the whole company at 5.9%.

That cash has swung, though. Free cash flow at Baxter fell from $1.29 billion in 2023 to $0.191 billion in the twelve months to March 2025, before recovering. Both of those earlier periods still include cash from Kidney Care, so they are not like-for-like with the latest figure.

Why Does The Market Doubt Baxter's Cash?

The share price appears to assume the cash will not last, and Baxter's thin reported profit gives a buyer reason to think so. Operating income of $0.264 billion over the latest twelve months fell short of the company's $0.298 billion interest bill. Its reported operating margin sat at 2.3%, against 7.2% two years earlier. That 2.3% includes an operating loss in fiscal Q4 2025. The latest quarter was better, landing at 7.6% against 6.1% a year earlier, and operating income of $0.225 billion covered that quarter's $0.074 billion of interest about three times.

Management said third-quarter free cash flow benefited in part from the timing of certain cash receipts, including a tariff refund and a legal settlement. Baxter will not get the same refund benefit in 2027, management said, and the transition services tied to Vantive, the Kidney Care business Baxter sold, should end by the end of January 2027.

Sales continue to grow, though: revenue rose 5.3% over the latest twelve months. Management raised its 2026 organic sales growth outlook to 3% to 4%, from 2% to 3%. So Baxter's sales are not shrinking, though its operating margin is thinner than two years ago and its debt is heavy.

Baxter Has Been Paying Down Its Debt

Total debt at Baxter stood at $8.81 billion at September 30, 2026, down from $13.2 billion two years earlier. During the third quarter, Baxter retired $864 million of debt through a cash tender offer. Management said it is more confident of reaching its target of about 3 times net leverage by the end of 2026.

The open question is whether Baxter's free cash flow keeps growing once the tariff refund and the legal settlement are behind it, while twelve-month operating income still falls short of the interest bill. Baxter's full-year free cash flow and its net leverage are the numbers to watch at its next report, expected in February 2027, alongside management's outlook for 2027. Until then, a shareholder can count on only part of the $1.13 billion, since some of it came from receipts that may not recur.
