Human Resources IRS 1095-C Notice

To reduce waste and promote efficiency, the City of Southfield is taking advantage of a new law that allows us to make your IRS Form 1095-C available to you only upon request. We are required to file this form with the IRS to show whether we offered you healthcare coverage in one or more months of calendar year 2025. If you recall, in the past, this form was mailed to you in the first quarter of the year. However, pursuant to the new law, we will not be mailing the forms this year.

While we are no longer automatically mailing the form to you, you can request a copy of your form at any time.
