In advance of the October 1, 2026 start of a requirement that hospices provide an addendum to EVERY hospice enrollee, the Centers for Medicare and Medicaid Services (CMS) on September 11, 2026 posted an updated Model Example of Hospice Election Statement and Model Example of the Hospice Election Addendum.
LeadingAge strongly advises members to download these new versions and make sure that providers' election statement and addendum are consistent with the agency's new model examples.
We know that not having consistent language on these documents can lead to denials of payments from Medicare Administrative Contractors (MACs).
