DCCA DISCIPLINARY ACTIONS

HONOLULU - The state Department of Commerce and Consumer Affairs (DCCA) and its respective state Boards and Commissions released a summary of disciplinary actions taken on individuals and entities with professional and vocational licenses in Hawai'i, through the month of July 2026. These disciplinary actions include dispositions based upon either the results of contested case hearings or settlement agreements submitted by the parties. Respondents enter into settlement agreements as a compromise to claims and to conserve on the expenses of proceeding with an administrative hearing.

The DCCA and the Boards and Commissions are responsible for ensuring those with professional and vocational licenses are performing up to the standards prescribed by state law.

HAWAIʻI MEDICAL BOARD

Respondent: Ronald J. Wong, M.D.

Case Number: MED 2025-9-L

Sanction: $1,000 fine, reprimand

Effective Date: 7-10-26

RICO alleges that on or about September 1, 2021, a medical malpractice suit was filed against Respondent in the state of Hawaiʻi. Despite the suit, on his renewal applications dated November 9, 2021 and November 6, 2023, Respondent answered "NO" to question number 4.1, "Since you last filed an application (initial/renewal) with the Board…have any claims of malpractice been filed against you?," in potential violation of HRS §§ 453-8(a)(6) and 453-8(a)(15). (Board approved Settlement Agreement.)

Respondent: James C. Cole, M.D.

Case Number: MED 2026-50-L

Sanction: $750 fine, condition of license

Effective Date: 7-10-26

RICO alleges that Respondent was disciplined by the states of Florida and Texas, in potential violation of HRS § 453-8(a)(11). (Board approved Settlement Agreement.)

Respondent: Louis T. House, M.D.

Case Number: MED 2026-0047-L

Sanction: Voluntary surrender of license

Effective Date: 7-10-26

RICO alleges that on or about December 10, 2025, Respondent entered into an Agreed Order with the Commonwealth of Kentucky Board of Medical Licensure (the "Kentucky Order"). RICO further alleges that the Kentucky Order concluded that based on the Stipulations of Fact, Respondent violated KRS 311.595(9) (engaged in dishonorable, unethical, or unprofessional conduct of a character likely to deceive, defraud, or harm the public or any member thereof), in potential violation of HRS §§ 453-8(a)(7), 453-8(a)(9), and 453-8(a)(11). (Board approved Settlement Agreement.)

Respondent: Atsushi Jim Terakubo, M.D.

Case Number: MED 2025-534-L

Sanction: $2,000 fine, reprimand

Effective Date: 7-10-26

RICO alleges that on or about May 16, 2023, Respondent was served with a medical malpractice suit. Despite the suit, on his renewal application dated January 24, 2024, Respondent answered "NO" to Question Number 4.1, asking if any claims of malpractice had been filed against him since he last filed an application, in potential violation of HRS §§ 453-8(a)(6) and 453-8(a)(15). (Board approved Settlement Agreement.)

Respondent: Roland Kuo-Yang Tang, M.D.

Case Number: MED 2026-0094-L

Sanction: Voluntary surrender of license

Effective Date: 7-10-26

RICO alleges that on or about January 2, 2026, the Medical Board of California entered its Decision against Respondent. RICO further alleges that Respondent did not contest the two (2) causes for discipline charged in the California Board's Accusation: 1) Repeated Negligent Acts; and 2) Failure to Maintain Adequate and Accurate Records, in potential violation of HRS §§ 436B-19(2), 436B-19(17), 453-8(a)(6), 453-8(a)(7), 453-8(a)(8), 453-8(a)(11), and 453-8(a)(15). (Board approved Settlement Agreement.)

Respondent: William B. Creel, M.D. aka Bradley Creel

Case Number: MED 2026-0054-L

Sanction: Reprimand, voluntary agreement not to restore or reinstate license and/or seek licensure

Effective Date: 7-10-26

RICO alleges that on or about May 13, 2025, a Criminal Information against Respondent was filed in the United States District Court of the Northern District of Georgia Atlanta Division. RICO further alleges that the Criminal Information charged that while aboard an aircraft, Respondent "did knowingly assault the Victim…". RICO further alleges that on or about May 15, 2025, Respondent entered a Guilty Plea and Plea Agreement, in which Respondent admitted that he was guilty of the crime(s) charged in the Criminal Information, in potential violation of HRS §§ 436-19(1), 436B-19(2), 436B-19(5), 436B-19(7), 436B-19(9), 436B-19(12), 436B-19(14), 436B-19(17), 453-8(a)(4), 453-8(a)(6), 453-8(a)(7), 453-8(a)(9), 453-8(a)(12), and 453-8(a)(15) and HAR §§ 16-85-112(1) and 16-85-112(3). (Board approved Settlement Agreement.)

Respondent: Sunil Kurup, M.D.

Case Number: MED 2025-474-L

Sanction: $2,000 fine

Effective Date: 7-10-26

RICO alleges that on March 21, 2025 the Texas Medical Board entered into an Agreed Order with Respondent which Respondent failed to report to the Board within 30 days, in potential violation of HRS § 453-8(a)(14). (Board approved Settlement Agreement.)

Respondent: David A. Sine, M.D.

Case Number: MED 2026-112-L

Sanction: $5,000 fine, prescribing prohibition

Effective Date: 7-10-26

RICO alleges that on or about December 9, 2024, Respondent was disciplined by the state of California, in potential violation of HRS § 453-8(a)(11). (Board approved Settlement Agreement.)

BOARD OF BABERING AND COSMETOLOGY

Respondents: Kelli L. Nguyen dba Rainbow Nails and Kelli L. Nguyen

Case Number: BAR 2025-123-L

Sanction: $1,500 fine

Effective Date: 2-10-26

RICO alleges that on or about June 19, 2025, Respondents permitted an unlicensed person to perform beauty operator activities for compensation, without the required beauty operator license, at Respondents' beauty shop. RICO further alleges that Respondents failed to conspicuously display their respective beauty shop and beauty operator licenses, in potential violation of HRS §§ 439A-3(a), 439A-16(a)(3), 439A-12, and 439A-16(a)(6). (Board approved Settlement Agreement.)

Respondents: New Nail & Beauty LLC dba Nail and Beauty, and Kristy B. Baoxin Peng

Case Number: BAR 2024-176-L

Sanction: $1,000 fine

Effective Date: 11-18-25

RICO alleges that on or about August 26, 2024, Respondents permitted two unlicensed persons to perform beauty operator activities for compensation in Respondents' beauty shop without the required beauty operator license, in potential violation of HRS § 439A-16(a)(3). (Board approved Settlement Agreement.)
