$3.5M to advance autonomous trauma robotics

Traumatic injury is a critical threat in large-scale combat operations, where survivable injuries can become fatal simply due to a lack of available surgeons. To address this severe capacity limitation, the Defense Advanced Research Projects Agency (DARPA) is launching the DARPA Surgical Competition (DSC), a $3.5 million prize competition designed to rapidly advance the state-of-the-art in autonomous trauma surgical robotics.

The DSC aims to spur the development of robotic systems that can act as skilled, reliable partners to human surgeons. Utilizing an "apprenticeship model," the competition will test emerging systems on their ability to collaboratively assist human surgeons during complex trauma procedures, and ultimately, independently execute lifesaving surgical interventions.

"In future conflicts or mass casualty incidents, our human medical capacity will be stretched beyond its limits," said Dr. Jeremy C. Pamplin, DARPA program manager for the DSC. "We are not trying to replace surgeons; we are building them a highly skilled partner. By pushing the boundaries of artificial intelligence, robotic dexterity, and human-machine teaming, we can create scalable, life-saving capacity when and where it is needed most."

The DSC will kick off officially on Sept. 22, 2026, with an Industry Day for potential performers and culminate with a Finals Competition Event in March 2027. Competitors will first need to pass rigorous Component Gates assessing Medical Knowledge, Physical Capability, and Rapid Learning.

Top performers will then advance to the operational lanes:

  • Surgical Collaboration Lane ($1.5M Prize Pool): Assessing platforms' ability to safely and efficiently assist a lead surgeon in a damage-control procedure.
  • Surgical Independence Lane ($1.5M Prize Pool): Evaluating platforms on their ability to autonomously assess, plan, and execute a trauma procedure without human intervention.
  • Component Gate Prizes: An additional $500,000 will be distributed to top performers in the individual sub-component capabilities.

By de-risking the technological barriers associated with open surgical robotics, DARPA also expects the technologies developed under this competition to offer profound dual-use benefits for civilian healthcare - from expanding specialist access in rural communities to bolstering disaster response capabilities.

Innovators in robotics, artificial intelligence, and bioengineering are encouraged to participate. The Biological Technologies Office (BTO) of DARPA will host an Industry Day for potential competitors in the DSC. The Industry Day will be held on Wednesday, Sept. 22, 2026.
