Independence Blue Cross (IBX) knows that the brain is at the center of everything a person does, and that keeping it healthy is essential to their well-being. Keeping this and the cognitive well-being of its members at the forefront, IBX has teamed up with Isaac Health, a virtual-first clinic that provides brain health and dementia care. Through this new relationship, a subset of IBX Medicare Advantage members who have a diagnosis of dementia or who are at risk for dementia have access to Isaac Health's medical and support services.
"Our goal is to help members who have a diagnosis of dementia address potential brain health concerns early and be connected with the appropriate resources for support," said Dr. Luz Ramos, geriatrician and medical director at Independence Blue Cross. "Working with Isaac Health helps us do exactly this and at the same time it empowers these members to take charge of their brain health and overall well-being. This is especially important, as the National Institutes of Health estimates that the aging of our population in the U.S. is expected to cause the number of new dementia cases per year to double by 2060."
Isaac Health offers a full range of medical and support services for people living with dementia and other brain health conditions. These services aim to improve quality of life for patients living with dementia and their family caregivers. The services that are included are:

Isaac Health's services are designed to be accessible to all who need it. For members with internet access, all medical consultations take place via telehealth. For those without internet access who may need extra support, Isaac Health arranges for an assistant to visit their home to help facilitate a virtual appointment.
IBX Medicare Advantage members who are eligible for these services can be referred to Isaac Health a few different ways. One way is through a referral from their primary care doctor or specialist. Another is if a member has worked with IBX Case Management due to a diagnosis of dementia or is at risk for dementia. In this case, Isaac Health's specialized team may reach out to them directly.
During outreach calls, Isaac Health specialists will introduce themselves as calling on behalf of IBX. They will also verify each member's identity for privacy and security purposes. Then they will ask a brief series of questions to determine if additional support or assessments are needed. If a member demonstrates cognitive changes in two or more areas during the initial assessment, they will have the option to schedule a follow-up virtual consultation with an Isaac Health provider.
